Refund Policy for Nutrition Services

TERMS AND POLICIES UPON PURCHASE

Upon purchase on the website, I agree to the following purchase terms and policies as well as others listed on the website.

CONSENT OF TREATMENT OF ADULT AND MINOR

Upon purchase, I provide consent for nutrition counseling. I also provide consent to treat my child, a minor if applicable. I am aware that I am responsible for notifying my or my child’s medical doctor before starting any nutritional program. I acknowledge this program does not replace care from my or my child’s current medical and/or healthcare practitioners. This program is based on nutritional testing, results, and educational and professional recommendations, such as but not limited to dietary changes and supplementation, and should be discussed with my medical doctor. I understand this program does not treat any health conditions.

WAIVE THE RIGHT TO A CONSULTATION AND/OR FOLLOW-UP VISIT

If applicable, I understand if I purchase a product or service on the website without a completed professional consultation and/or follow-up visit, I am waiving my rights to have Dr. Sulsenti fully review my health concern, medical history, and any previous lab reports so she can make a professional recommendation regarding the best nutritional path for my needs. This may or may not include the purchase of labs, supplements, and/or nutrition counseling or programs. I take full responsibility for the labs/purchase and accept any consequences that may arise from doing so.

LAB ANALYSIS WITH ONE IN-OFFICE OR TELEMEDICINE LAB REVIEW VISIT

Lab purchases include a complete lab analysis by Dr. Suslenti, followed by a thorough one-lab review visit in the office or via telemedicine with Dr. Sulsenti. I understand that during this visit, I will receive lab result interpretations and an individualized nutrition management plan with professional recommendations. This may include dietary changes, supplementation, and protocols specific to results. I also understand that her interpretations may be limited if I have waived the consultation and/or follow-up visit. I accept that Dr. Sulsenti does not provide multiple one-on-one reviews for multiple tests I may purchase separately or in a bundle; all lab tests will be evaluated together in her one review visit for optimal results.

CANCELLATION POLICY

There is a $50 cancellation policy and No-Show Fee for all functional medicine and nutrition visits with Dr. Sulsenti. We take your health and time seriously, dedicating time solely for you and no other patients. Dr. Sulsenti prepares ahead for your visit(s) and is committed to the time allocated just for you. If a visit is canceled or missed, it cannot be rescheduled without the Cancellation and No-Show Fee being paid in full.

REFUND POLICIES, PAYMENT AGREEMENTS, AND CANCELLATIONS OF PURCHASE

I understand there are no full refunds. I recognize that my purchase includes fees for various services such as, but not limited to, patient email/office support, lab processing, lab scheduling, patient processing, lab visits on-premises and via telemedicine, visit preparations, menu preparations, lab report processing and preparation, nutrition counseling visits, and all nutrition support inside and outside of visits (questions, phone calls, texting, emails, etc.), outlines, menus, recipes, nutrition management plans, lab reports, and more, provided by Dr. Lisa and her staff. I understand that I begin nutrition services with Dr. Lisa and her staff upon purchasing a lab service on this website.

If I choose to cancel my purchase, I must notify Dr. Lisa and her staff in writing with reasons for my decision. I understand there is a $200 non-refundable nutrition scheduling, session preparation, and lab processing fee. I acknowledge that all services rendered, including but not limited to those mentioned above, are not refundable, nor are any supplements or physical products purchased. There may be an additional cancellation fee associated with Vibrant America for lab cancellations, alongside credit card processing fees. Dr. Sulsenti and her staff will strive to reimburse for any canceled labs and orders; however, full reimbursement will not be possible due to the aforementioned fees and explanations.

COPYRIGHT LAWS

I acknowledge I am responsible for how I use Dr. Lisa Sulsenti’s nutritional information, such as but not limited to lab forms, lab kits, lab testing and results, leaky gut testing, all food menus, recipes, nutrition management plans, and supplement recommendations. I will not reproduce or administer any information, such as lab forms, lab results, diets, menus, recipes, handouts (printed or digital files), or professional management plans to others for personal or business purposes. If I share a recipe, I will give credit to Dr. Lisa Sulsenti at all times. I understand that this program is the sole property of Dr. Lisa Sulsenti and that violating these rules may infringe upon copyright laws punishable under the law.

PRIVACY, TRANSMISSION, AND RELEASE OF PATIENT INFORMATION POLICIES

I agree to read and understand my rights under the Health Insurance Portability and Accountability Act (HIPAA) privacy laws that will be provided. I also give Dr. Lisa Sulsenti permission to transmit my nutritional and medical information, including consultations, health discussions, lab results, history, menus, nutrition management reports, and recommendations via email, telemedicine, phone calls, fax, and text. I will provide her with emails, numbers, and handles for sending my information, and I will inform her 48 hours in advance of any changes to this agreement in writing.
